Job Description

A. Community Outreach & Household Engagement

• Conduct household visits in assigned villages for screening, data collection, follow-up, and basic counselling
• Provide simple, family-level guidance on cardiovascular disease prevention & management

B. Screening & Service Delivery Support

• Conduct basic health measurements /assessments
• Facilitate consultations between households and doctors’ teams through digital platforms
• Help ensure timely referrals and follow-up as guided by the site team

C. Digital Health & Data Entry

• Enter household-level information in tablets/mobile apps accurately
• Support basic digital literacy at community level (e.g., ABHA ID, simple app navigation)
• Maintain daily and weekly activity logs in simple formats (Word/Excel/PPT templates will be provided)

D. Coordination & Reporting

• Coordinate with project team on site and at the central level
• Share regular updates, reports, and field insights with the site team
• Participate in all required trainings, review meetings, and supervision visits

E. Any Other Duties as assigned, based on emerging needs
