We seek a compassionate, certified Family Medicine Specialist to provide comprehensive, continuous care for patients across the lifespan. The clinician will diagnose and manage acute and chronic medical conditions, promote preventive health, and collaborate with multidisciplinary teams to ensure high-quality, patient- and family-centered care.

Key Responsibilities:

1. Provide medical assessment, diagnosis, treatment, and follow-up for patients of all ages, from infants to older adults.
2. Conduct preventive care visits, chronic disease management, immunizations, health screenings, and lifestyle counseling.
3. Manage acute illnesses and common emergencies in the primary care setting; stabilize and refer complex cases to appropriate specialists or higher-level care as needed.
4. Develop and implement individualized, evidence-based care plans for chronic conditions (eg, diabetes, hypertension, heart disease, mental health).
5. Perform minor procedures within the scope of family medicine as required.
6. Communicate effectively with patients and families regarding diagnoses, treatment options, and prognosis; obtain informed consent and support shared decision-making.
7. Coordinate care with nurses, specialists, therapists, and allied health professionals; participate in multidisciplinary case conferences and referral planning.
8. Maintain accurate, timely clinical documentation in the electronic health record and complete required administrative tasks (prescriptions, referrals, medico-legal forms).
9. Participate in quality improvement, clinical governance, continuing professional development, teaching of trainees (if any), and community health initiatives, including outreach and preventive programs.
10. Adhere to hospital policies, clinical guidelines, infection control, prescribing regulations, and safeguarding protocols.
11. Provide on-call coverage or urgent-care clinic shifts as rostered.
