Description
- Conduct comprehensive assessments of patients with serious, life-limiting illnesses to understand their physical, emotional, and spiritual needs.
- Develop and implement plans for managing symptoms such as pain, nausea, and fatigue, with a focus on improving the patient's comfort and quality of life.
- Facilitate open and honest communication with patients and their families regarding prognosis, treatment options, and end-of-life decisions.
- Collaborate with the primary medical team, specialists, nurses, and other healthcare providers to ensure a coordinated and holistic approach to patient care.
- Assist patients in creating advance care directives and making decisions about goals of care, including end-of-life preferences.
- Provide emotional and psychosocial support to patients and their families, addressing the psychological and spiritual aspects of coping with a serious illness.
- Work as part of an interdisciplinary team, including social workers, chaplains, nurses, and other specialists, to address the various needs of patients and their families.
- Provide education to patients, families, and other healthcare professionals about palliative care principles, symptom management, and end-of-life care.
- Excellent communication skills to facilitate discussions about sensitive topics and complex medical issues.
