Description

Conduct comprehensive assessments of patients with serious, life-limiting illnesses to understand their physical, emotional, and spiritual needs.
Develop and implement plans for managing symptoms such as pain, nausea, and fatigue, with a focus on improving the patient's comfort and quality of life.
Facilitate open and honest communication with patients and their families regarding prognosis, treatment options, and end-of-life decisions.
Collaborate with the primary medical team, specialists, nurses, and other healthcare providers to ensure a coordinated and holistic approach to patient care.
Assist patients in creating advance care directives and making decisions about goals of care, including end-of-life preferences.
Provide emotional and psychosocial support to patients and their families, addressing the psychological and spiritual aspects of coping with a serious illness.
Work as part of an interdisciplinary team, including social workers, chaplains, nurses, and other specialists, to address the various needs of patients and their families.
Provide education to patients, families, and other healthcare professionals about palliative care principles, symptom management, and end-of-life care.
Excellent communication skills to facilitate discussions about sensitive topics and complex medical issues.
