Description

Receives patients, carries out complete assessment of assigned patients on admission / at the beginning of the shift, summarizes data and states nursing diagnoses/ patient care needs.
Carries out nursing interventions to assigned patients on priority basis in accordance with the stated medical plan of care consistent with the established policies and procedures of the department.
Evaluates the effectiveness of care provided by reassessing patients as required and amends plan of care to achieve best results.
Performs admission, discharge and transfer procedures, and assists others with patient care activities
Observes and records signs, symptoms, and behaviors, including the physiological status of patients; reports assessment changes, to the concerned authority.
Practices infection controls measures to protect patient form acquiring or spreading infection.
Demonstrates awareness of potential/actual risks of infection and applies appropriate preventive measures.
Ensures continuity of care through clear and concise (verbal and written) communication.
Executes physician's orders for all assigned patients; safely and properly administers medications and treatments.
Maintains patient’s hygiene and health.
Carries out the prescribed investigations for the correct patients and informs findings to the physician.
Prepares assigned patients for planned procedures and surgeries.
Participates in clinical rounds, provides relevant information as regards to nursing observations made on individual patients and updates changes/special instructions in the patient's record.
Carries out changes / special instructions in the plan of care as advised by the Consultant /doctor on duty.
Assists for procedures as required at the bedside.
Completes documentation of assessment findings, investigations done, procedures carried out and the treatment given to the patient during the shift according to hospital protocol.
Ensures availability of adequate supply of medicines and consumables with the patient and return of excess materials/drugs to the respective department
Maintains cleanliness of assigned patient’s units and the department.
Handles all patient care equipment with care and reports any damages/ change in working condition of the equipment to the Nurse Supervisor / Team Leader / Nurse In charge.
Ensures that all items used for patient care inclusive of medications, surgical supplies are duly recorded and charged.
Ensures patient’s safety at all times by taking necessary preventive measures Checks the assigned inventory in all shifts and informs the Nurse Supervisor /Team Leader/ Nurse In charge in case of any discrepancies noted.
Ensures the completion of shift report and endorses the patients to the next duty assigned staff.
Ensure s that patient's and or family's needs are met with priority and or communicated to the concerned department/person Educates patient and family members regarding the preventive aspects of care / discharge plans.
Ensures the readiness of patient for discharge – collection and verification of reports to be handed over to the patient/ indenting the discharge medications, arranging for continuity of care at home, transport etc. in coordination with the Nurse Supervisor/Senior Nurse /Team Leader / Nurse In charge on duty.
Ensures timely discharge/transfer of patients and keeps the discharged patient's unit prepared to receive new admission/transfer.
