Description
- Assess patients' physical, emotional, and psychological needs through regular examinations and conversations.
- Develop individualized care plans in collaboration with the medical team to address patient needs and conditions.
- Provide direct patient care, including administering medications, wound care, and treatments as prescribed by the physician.
- Monitor patient responses to treatments, medications, and procedures, and adjust care plans as needed.
- Ensure patient comfort by addressing concerns, providing emotional support, and explaining medical procedures.
- Administer medications and treatments as ordered by physicians, ensuring correct dosages and proper techniques.
- Assist physicians and medical staff in performing medical procedures and diagnostic tests.
- Maintain accurate and up-to-date patient records, including assessments, care plans, and progress notes.
- Educate patients and their families about prescribed treatments, medication regimens, and home care instructions.
- Monitor vital signs (e.g., blood pressure, heart rate, temperature, respiratory rate) regularly and report changes to the medical team.
- Observe and document any changes in a patient’s condition and notify the healthcare provider as needed.
- Track patients' progress and assist in modifying care plans based on medical evaluations.
