Description

Conduct comprehensive geriatric assessments, including medical, functional, cognitive, and psychosocial evaluations, to identify patients' individual needs, preferences, and risks.
Diagnose, treat, and manage a wide range of age-related health conditions and geriatric syndromes, including dementia, osteoporosis, cardiovascular disease, diabetes, and mobility issues.
Develop personalized care plans tailored to each patient's unique needs, goals, and preferences, integrating evidence-based medical interventions, lifestyle modifications, and supportive services.
Coordinate care with interdisciplinary healthcare teams, including nurses, therapists, social workers, and specialists, to provide holistic and coordinated care to elderly patients.
Monitor patients' progress and response to treatment, adjust care plans as needed, and provide ongoing support, education, and counseling to patients and their families.
Manage complex medication regimens, optimize medication use, and minimize polypharmacy while considering the pharmacokinetics and pharmacodynamics in elderly patients.
Provide palliative and end-of-life care to patients nearing the end of life, addressing symptom management, advance care planning, and psychosocial support needs.
Promote preventive care and health promotion strategies, including immunizations, cancer screenings, nutrition counseling, exercise prescription, and fall prevention programs, to optimize the health and well-being of elderly patients.
