Description

Candidate should be Graduate/Postgraduate preferably in Medical Social Work (MSW) and should have Pleasing personality and prolific in Patient / Customer / Client / Public Relationship Management.
Conduct comprehensive psychosocial assessments of patients and families, evaluating their social, emotional, and environmental circumstances, strengths, and challenges that may affect their health and medical care.
Identify psychosocial risk factors, such as substance abuse, mental health issues, socioeconomic challenges, and family dynamics, and assess their impact on patient health and treatment adherence.
Provide individual, family, and group counseling to patients and families, addressing issues related to coping with illness, adjusting to medical diagnoses, managing stress, and enhancing resilience.
Offer emotional support, validation, and crisis intervention to patients and families facing difficult life situations, such as chronic illness, terminal illness, grief, loss, or trauma.
Collaborate with healthcare providers, interdisciplinary team members, and community resources to develop and implement care plans that address the psychosocial needs of patients and families.
Advocate for patients' rights and access to healthcare services, helping them navigate the healthcare system, obtain needed resources, and overcome barriers to care.
Identify and coordinate access to community resources and support services, including financial assistance, housing resources, transportation assistance, home health services, and mental health counseling.
