Description

Dispense prescription medications accurately and safely, ensuring compliance with legal and regulatory requirements, pharmacy policies, and medication management protocols.
Provide drug information and counseling to healthcare professionals, patients, and caregivers regarding medication indications, dosages, interactions, side effects, and precautions.
Perform medication therapy reviews, patient assessments, and medication reconciliation to identify and resolve drug-related problems, improve medication adherence, and prevent medication errors.
Monitor medication orders, drug interactions, and potential adverse drug reactions, and intervene as needed to optimize medication therapy and patient safety.
Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care, promote medication safety, and facilitate medication management processes.
Maintain accurate and up-to-date patient records, medication profiles, and documentation of pharmacy services in electronic health records (EHR) or pharmacy information systems.
Provide immunization services, medication counseling, and medication therapy management (MTM) services to patients as needed, in accordance with state pharmacy laws and regulations.
Participate in pharmacy quality assurance activities, medication safety initiatives, and continuous quality improvement efforts to enhance pharmacy services and patient outcomes.
Stay informed about new drugs, drug therapies, and pharmaceutical developments through participation in continuing education programs, pharmacy conferences, and professional development activities.
