Description
- Dispense prescription medications accurately and efficiently, following legal and ethical guidelines, and verifying prescription information for completeness and accuracy.
- Provide medication counseling to patients on proper medication use, dosage instructions, potential side effects, drug interactions, and storage requirements.
- Review medication orders and patient profiles to ensure appropriateness of therapy, identify potential drug-related problems, and collaborate with healthcare providers to optimize medication regimens.
- Assess patient health records and medical history to identify drug allergies, contraindications, and other relevant information that may impact medication therapy.
- Monitor and manage medication therapy for patients with chronic conditions, ensuring adherence to treatment plans, evaluating therapeutic outcomes, and making appropriate recommendations for adjustments or interventions.
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care, resolve medication-related issues, and promote interdisciplinary communication and teamwork.
- Perform medication reconciliation and review medication lists during transitions of care to prevent medication errors and adverse drug events.
- Maintain accurate and up-to-date patient records, including medication profiles, drug allergies, and medication histories, using electronic health records (EHR) and pharmacy management systems.
