Description

Welcome patients and their families upon arrival, providing assistance with registration, admission procedures, and completion of necessary forms.
Conduct initial assessments of patients' needs, preferences, and medical histories, gathering relevant information to facilitate personalized care planning.
Coordinate patient admissions, transfers, and discharges, ensuring smooth transitions between different care settings and departments within the healthcare facility.
Serve as a primary point of contact for patients and their families, addressing inquiries, providing information about services, and offering emotional support and reassurance as needed.
Collaborate with healthcare providers, including physicians, nurses, and allied health professionals, to coordinate patient care activities, treatments, and interventions.
Monitor patients' condition and vital signs regularly, reporting any changes or concerns to the appropriate healthcare team members and taking prompt action as necessary.
Assist with activities of daily living (ADLs) for patients, including bathing, dressing, feeding, and mobility support, maintaining their comfort and dignity at all times.
Administer medications and treatments as prescribed by healthcare providers, following established protocols and ensuring accurate documentation of medication administration.
