Description
- Coordinate patient care activities and ensure the delivery of high-quality, evidence-based nursing care in accordance with established standards and protocols.
- Assess patient acuity and care needs, develop individualized care plans, and monitor patient progress throughout the healthcare continuum.
- Collaborate with interdisciplinary teams, including physicians, therapists, and ancillary staff, to facilitate coordinated patient care and achieve optimal outcomes.
- Supervise and lead nursing staff, including Registered Nurses (RNs), Licensed Practical Nurses (LPNs), and Certified Nursing Assistants (CNAs), providing guidance, support, and feedback as needed.
- Delegate nursing tasks and responsibilities effectively, ensuring appropriate assignment of duties based on staff competency, scope of practice, and patient care needs.
- Conduct regular performance evaluations, provide ongoing coaching and mentoring, and address performance issues or concerns in a timely and constructive manner.
- Provide clinical leadership and expertise to nursing staff, serving as a resource for clinical decision-making, problem-solving, and nursing practice issues.
- Participate in nursing rounds, interdisciplinary rounds, and patient care conferences to contribute nursing perspectives, identify care coordination needs, and promote collaboration among team members.
- Monitor compliance with nursing practice standards, regulatory requirements, and organizational policies, addressing deviations and implementing corrective actions as necessary.
