Description

Create, update, and maintain patient medical records in accordance with established policies, procedures, and regulatory requirements, ensuring accuracy, completeness, and confidentiality of information.
Organize and index medical records, both electronically and in paper format, using appropriate coding and classification systems to facilitate efficient storage, retrieval, and tracking of records.
Enter patient demographic information, medical history, diagnoses, treatments, and procedures into electronic health record (EHR) systems or other health information management (HIM) software accurately and in a timely manner.
Assign appropriate diagnostic and procedural codes, such as ICD-10 (International Classification of Diseases, 10th Edition) and CPT (Current Procedural Terminology) codes, to patient encounters for billing, reimbursement, and statistical purposes.
Review medical records for completeness, accuracy, and compliance with documentation standards and regulatory requirements, identifying and resolving discrepancies or deficiencies as needed.
Conduct regular audits and quality assurance checks of medical records, ensuring data integrity, adherence to coding guidelines, and compliance with privacy and security regulations, such as HIPAA (Health Insurance Portability and Accountability Act).
Retrieve and release medical records to authorized individuals or entities in accordance with patient privacy laws and organizational policies, responding to requests from healthcare providers, patients, insurance companies, and legal authorities promptly and accurately.
Maintain logs and documentation of record requests, disclosures, and releases, ensuring proper documentation and tracking of all record-related activities.
