Description
- Conduct thorough psychosocial assessments of patients and their families to identify strengths, challenges, and support needs.
- Assess patients' emotional, social, cultural, and financial factors impacting their health and well-being.
- Utilize standardized assessment tools, interviews, and observations to gather relevant information and develop a comprehensive understanding of patients' situations.
- Develop individualized care plans in collaboration with patients, families, and interdisciplinary teams to address identified psychosocial needs and goals.
- Provide counseling, crisis intervention, and supportive services to patients and families facing medical crises, life transitions, or end-of-life care.
- Advocate for patients' rights, preferences, and access to healthcare services, including assistance with advanced directives, guardianship, and healthcare decision-making.
- Coordinate with community agencies, social service organizations, and healthcare providers to facilitate access to resources such as housing, financial assistance, transportation, and mental health services.
- Provide referrals and follow-up support to ensure continuity of care and access to needed services beyond the healthcare setting.
- Assist patients and families in navigating healthcare systems, understanding insurance coverage, and accessing entitlement programs and benefits.
