Description
- They help patients understand their illness or condition and provide them with information about the resources available to them to cope with the emotional, financial, and social needs that arise with a diagnoses.
- Conduct psychosocial assessments of patients and families to identify strengths, needs, and resources for coping with medical conditions, treatment, and healthcare decisions.
- Provide counseling, emotional support, and crisis intervention to patients and families facing medical crises, chronic illness, disability, or end-of-life care.
- Develop individualized care plans in collaboration with interdisciplinary healthcare teams to address psychosocial issues, improve coping skills, and enhance patient well-being.
- Coordinate referrals to community resources, support services, and financial assistance programs to meet the holistic needs of patients and families.
- Advocate for patients' rights, preferences, and quality of life throughout the healthcare continuum, including hospitalization, rehabilitation, and home care.
- Facilitate communication and decision-making among patients, families, and healthcare providers to ensure informed consent, advance care planning, and goal-directed care.
- Collaborate with case managers, discharge planners, and other healthcare professionals to facilitate transitions of care, discharge planning, and continuity of services.
