Job Description

  1. Maintain, organize, and secure patient medical records (OPD & IPD).
  2. Ensure accurate documentation of medical histories, treatment, and procedures.
  3. Code diagnoses and procedures using ICD standards.
  4. Handle admission, discharge, and transfer (ADT) records.
  5. Ensure compliance with NABH / JCI / Hospital policies.
  6. Audit medical records for completeness and accuracy.
  7. Manage electronic medical records (EMR/EHR) systems.
  8. Prepare reports for clinical, legal, and administrative use.
  9. Maintain confidentiality of patient information at all times.
  10. Coordinate with doctors, nurses, billing, and insurance teams.
  11. Support medical audits, insurance claims, and medico-legal cases.
