  • Data Entry and Maintenance: Input and update patient information, including demographics, medical histories, and treatment details, into electronic health record (EHR) systems, ensuring data integrity.
  • Compliance and Regulation: Ensure that all medical records comply with relevant legal, regulatory, and institutional policies, including HIPAA guidelines for patient confidentiality and data security.
  • Coordination with Healthcare Staff: Collaborate with physicians, nurses, and administrative staff to collect and verify patient information, facilitating accurate and timely documentation of care provided.
  • Quality Assurance: Conduct regular audits of medical records for accuracy and completeness, identifying and resolving discrepancies, and ensuring compliance with quality standards.
  • Release of Information: Process requests for patient medical records from authorized parties, ensuring appropriate consent is obtained and that information is shared in accordance with legal and institutional guidelines.
  • Record Retention and Archiving: Implement procedures for record retention and archiving, ensuring that medical records are preserved for the required duration and are easily retrievable when needed.
