Job Overview:

As a Hospital Pharmacist, you will be responsible for ensuring the safe, effective, and efficient use of medications within the hospital setting. You will work closely with healthcare professionals to provide pharmaceutical care to patients and ensure compliance with relevant laws, regulations, and standards.

Key Responsibilities:

1.Dispensing Medications:

  • Receive, interpret, and process medication orders accurately and efficiently.
  • Prepare and dispense medications in accordance with prescriptions, ensuring accuracy in dosage, labeling, and packaging.
  • Maintain proper documentation of medication dispensing and patient records.

2. Medication Management:

  • Review medication orders for appropriateness, including dosage, route, and frequency, and ensure compatibility with patient's medical conditions and other medications.
  • Provide drug information and counsel patients and healthcare providers on medication use, side effects, and interactions.
  • Collaborate with physicians and other healthcare professionals to optimize medication therapy and ensure patient safety and efficacy.

3.Drug Distribution and Inventory Management:

  • Oversee the procurement, storage, and distribution of medications and pharmaceutical supplies.
  • Monitor medication inventory levels, expiration dates, and storage conditions, and take appropriate actions to ensure adequate supply and minimize waste.
  • Participate in inventory control activities, such as stock rotation, ordering, and managing shortages.

4.Quality Assurance and Regulatory Compliance:

  • Implement and maintain quality assurance programs to ensure compliance with regulatory requirements, accreditation standards, and best practices.
  • Participate in medication safety initiatives, error reporting, and root cause analysis to prevent medication errors and adverse drug events.
  • Stay updated on changes in regulations, guidelines, and pharmacotherapy trends, and incorporate them into pharmacy practice.

5.Interdisciplinary Collaboration:

  • Work collaboratively with other healthcare professionals, including physicians, nurses, and allied health staff, to optimize patient care outcomes.
  • Participate in interdisciplinary rounds, committees, and clinical conferences to contribute pharmacy expertise and coordinate patient care.

6.Education and Training:

  • Provide education and training to pharmacy staff, healthcare providers, and patients on medication-related topics, including new medications, drug interactions, and medication safety.
  • Serve as a preceptor for pharmacy students, interns, and residents, and facilitate their learning experiences within the hospital pharmacy setting.

Qualifications:

  • Bachelor's or Master's of pharmacy (B.Pharma / M.Pharma) degree from an accredited pharmacy program.
  • Current pharmacist licensure in the state of practice.
  • Completion of a pharmacy residency program (preferred).
  • Clinical experience in a hospital or healthcare setting.
  • Strong knowledge of pharmacotherapy principles, medication management practices, and regulatory requirements.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work effectively in a fast-paced, interdisciplinary team environment.
  • Commitment to continuous learning and professional development.

Working Conditions:

  • The Hospital Pharmacist typically works in a hospital or healthcare facility environment.
  • May be required to work evening, weekend, or holiday shifts on a rotating basis.
  • May involve standing for extended periods and lifting or moving heavy objects.
