- Conduct initial and ongoing assessments of the patient's physical, emotional, and social needs.
- Evaluate the effectiveness of care plans and adjust as necessary in consultation with doctors and family members.
- Administer medications as prescribed, ensuring adherence to schedules.
- Perform wound care, manage catheter or colostomy bags, and other specific healthcare procedures as needed.
- Assist with feeding, ensuring patients meet their nutritional needs.
- Educate patients and their families on proper care techniques, medications, diet, and exercise.
- Collaborate with family members to involve them in the care plan, ensuring seamless continuity of care.
