Description

Conduct comprehensive assessments of patients' health status, including physical, emotional, and psychosocial aspects, to develop individualized care plans and goals.
Provide skilled nursing care to patients in their homes, including but not limited to wound care, medication management, IV therapy, catheter care, and palliative care as needed.
Monitor patients' condition and response to treatment, and collaborate with physicians and other healthcare professionals to adjust care plans and interventions as necessary.
Educate patients and their families on disease management, medication administration, nutrition, safety precautions, and other aspects of self-care to promote optimal health and independence.
Administer medications and treatments as prescribed by healthcare providers, ensuring accuracy, safety, and adherence to established protocols and regulations.
Coordinate with interdisciplinary team members, including therapists, social workers, and home health aides, to ensure coordinated and holistic patient care.
Document all patient care activities, assessments, and interventions accurately and timely in electronic health records (EHR) or other designated systems, in compliance with regulatory standards and agency policies.
Provide emotional support and counseling to patients and their families, addressing concerns, providing resources, and facilitating access to community services as needed.
