Responsibilities include the following and other duties may be assigned <ul><li>Explain the features of the Relex Medication Therapy Management Program to prospective or existing customers/patients</li><li>Effect medication replenishment, dosage tracking and obtain patient feedback.</li><li>Monitor outcomes and follow up on reminders to patient against counselling notes from Clinical Pharmacist and required lab tests</li><li>First level patient counselling and guidance on correct use of medication for achieving desired/targeted outcomes</li><li>Communicate customer feedback and customer experience on products/services and/or modifications to existing products/services to Operations, Marketing and Quality teams.</li><li>Support customer outreach programs through activities associated with theme of the program</li><li>Provide information and explanation of diagnoses, options, outcome and risk and escalate where required</li><li>Data updation in all tracking platforms on a daily basis</li></ul>
