We Apollo Home Healthcare Mumbai is putting up this proposal with you to Empaneled with the Organization and provide services (Physical/Virtual) on call basis to patients at Home.

An agreement shall be made and remain valid unless & until terminated by providing notice by either party.

AED shall get 70% Revenue share per visit if patient identified by Apollo Homecare (No conveyance shall be borne by Apollo). TDS will be applicable as per TDS norms.

Payment for the services delivered during calendar month will be paid by 10th of subsequent month.

Location: Nashik

Roles and responsibilities:

·        Home visits, visit approximately 4 to 5 enrolled patients in a day.

·        To assess the patients needs and formulate personalized care plan based on comprehensive assessment

·        To deliver outstanding clinical care guided by evidence based medicine and ethical practices

·        To monitor the care delivered and provide the necessary clinical leadership to facilitate better recovery

·        To provide Goal based care for patients resulting in better outcomes

·        To implement clinical care protocols and pathways for common home healthcare presentations

·        Report on number of visits and maintain the documentation of the visits
