Description
- Provide specialized nephrology consultations and comprehensive medical care to patients with acute and chronic kidney diseases, including evaluation, diagnosis, treatment, and follow-up management.
- Perform thorough patient assessments, including medical history review, physical examinations, and interpretation of laboratory tests, imaging studies, and renal biopsy results, to formulate accurate diagnoses and treatment plans.
- Develop and implement individualized treatment plans for patients with kidney disorders, incorporating evidence-based medical therapies, lifestyle modifications, and renal replacement therapies (hemodialysis, peritoneal dialysis) as appropriate.
- Manage patients with end-stage renal disease (ESRD) requiring renal replacement therapy, including initiation, maintenance, and monitoring of dialysis treatments, and evaluation for renal transplantation candidacy.
- Participate in the evaluation, selection, and management of renal transplant recipients and living kidney donors, collaborating with transplant surgeons, immunologists, and transplant coordinators to optimize transplant outcomes.
- Coordinate care for patients undergoing kidney transplantation, including pre-transplant assessment, immunosuppressive therapy management, post-transplant monitoring, and long-term follow-up care.
- Conduct outpatient nephrology clinics, providing ongoing monitoring, medication management, and education for patients with kidney disease, hypertension, electrolyte disorders, and other nephrological conditions.
- Collaborate with multidisciplinary healthcare teams, including primary care physicians, nephrology nurses, dietitians, social workers, and pharmacists, to provide holistic patient care and address patients' medical, psychosocial, and nutritional needs.
