Review and assess patient medication regimens, ensuring that prescribed treatments align with best practices, clinical guidelines, and individual patient needs.
Make recommendations to healthcare providers regarding appropriate drug choices, dosage adjustments, and medication schedules based on patient conditions and therapeutic responses.
Monitor patients' responses to medications, identifying potential adverse drug reactions, drug interactions, or non-compliance.
Collaborate with medical and nursing staff to optimize pharmacological therapy and ensure that drugs are being used effectively and safely.
Study and assess how drugs are absorbed, distributed, metabolized, and excreted in the body (pharmacokinetics), and their physiological effects (pharmacodynamics).
Adjust drug doses and therapy based on individual patient factors, such as age, weight, kidney and liver function, and concurrent diseases.
Monitor, report, and help prevent medication errors, ensuring that proper protocols are followed in drug administration.
Provide guidance on managing medication-related risks and enhancing patient safety in the healthcare setting.
