Description

  • Review and assess patient medication regimens, ensuring that prescribed treatments align with best practices, clinical guidelines, and individual patient needs.
  • Make recommendations to healthcare providers regarding appropriate drug choices, dosage adjustments, and medication schedules based on patient conditions and therapeutic responses.
  • Monitor patients' responses to medications, identifying potential adverse drug reactions, drug interactions, or non-compliance.
  • Collaborate with medical and nursing staff to optimize pharmacological therapy and ensure that drugs are being used effectively and safely.
  • Study and assess how drugs are absorbed, distributed, metabolized, and excreted in the body (pharmacokinetics), and their physiological effects (pharmacodynamics).
  • Adjust drug doses and therapy based on individual patient factors, such as age, weight, kidney and liver function, and concurrent diseases.
  • Monitor, report, and help prevent medication errors, ensuring that proper protocols are followed in drug administration.
  • Provide guidance on managing medication-related risks and enhancing patient safety in the healthcare setting.
