Job Description

  1. Review prescriptions & ensure safe, appropriate medication use.
  2. Participate in daily clinical rounds and provide drug-related recommendations.
  3. Monitor drug interactions, side effects & report adverse drug reactions.
  4. Educate patients and caregivers on proper medication use.
  5. Maintain hospital formulary and assist in drug selection.
  6. Coordinate with doctors, nurses & other healthcare professionals.
  7. Conduct medication reconciliation for admissions & discharges.
  8. Maintain records of clinical interventions & pharmacy audits.
  9. Ensure compliance with hospital, NABH, and regulatory standards.
  10. Support pharmacy-related research, audits, and staff training.
