Job Description
- Review prescriptions & ensure safe, appropriate medication use.
- Participate in daily clinical rounds and provide drug-related recommendations.
- Monitor drug interactions, side effects & report adverse drug reactions.
- Educate patients and caregivers on proper medication use.
- Maintain hospital formulary and assist in drug selection.
- Coordinate with doctors, nurses & other healthcare professionals.
- Conduct medication reconciliation for admissions & discharges.
- Maintain records of clinical interventions & pharmacy audits.
- Ensure compliance with hospital, NABH, and regulatory standards.
- Support pharmacy-related research, audits, and staff training.
