Description

Review and assess patients' medication plans to ensure safety, efficacy, and appropriateness based on clinical guidelines and individual patient needs.
Adjust or recommend modifications to medication regimens to minimize side effects and enhance therapeutic outcomes.
Educate patients and their families about the proper use of medications, potential side effects, and adherence to therapy.
Provide counseling on lifestyle adjustments, dietary considerations, and self-monitoring techniques to support medication effectiveness.
Monitor and evaluate potential drug interactions, allergies, and adverse effects to prevent medication-related complications.
Promptly intervene or report when adverse drug reactions occur, adjusting therapy as needed in collaboration with the healthcare team.
Work closely with physicians, nurses, and other healthcare professionals to develop and implement individualized treatment plans.
Participate in multidisciplinary rounds and case reviews to contribute pharmacologic expertise and ensure cohesive patient care.
Maintain accurate and detailed records of patient medication histories, treatment plans, interventions, and outcomes.
Ensure compliance with regulatory standards, hospital policies, and medication safety protocols.
Stay updated on advancements in pharmacology, new drug approvals, and emerging treatment guidelines.
Participate in or conduct clinical research studies, contributing to evidence-based practices within the healthcare setting.
Assist in the management of the hospital or clinic formulary by recommending cost-effective therapies and managing inventory.
Advocate for generic alternatives when clinically appropriate to support patient access and manage healthcare costs.
Provide training and education to healthcare staff on pharmacotherapy, medication safety, and adverse effect management.
Organize workshops or seminars on the latest developments in pharmaceutical care.
