Medication Therapy Management: Conduct comprehensive medication reviews and assessments to develop, implement, and monitor individualized medication therapy plans for patients.
Patient Counseling and Education: Provide education to patients about their medications, including proper usage, potential side effects, and drug interactions, empowering them to manage their health effectively.
