Description

  • Provide professional nursing care with an assigned unit according to established standards and procedures.
  • Provide comprehensive care to assigned patients.
  • Follows policy and delivers designated nursing interventions to assigned patients that are consistent with the documented medical plan of care.
  • Takes the nursing history, summarizes data and states nursing diagnoses / patient care needs.
  • Organize and maintain patient file and reports as per the MRD requirements in all shifts.
  • Ensure accurate documentation in nursing assessment sheet, pre and post-operative documents, intake output chart, ER chart, ICU chart etc. Don’t use other ink to erase, only strike off with single line.
  • Always greet with smile to patient and companion for their well-being.
  • Be groomed always.
  • Observes and records signs, symptoms and behaviours including the physiological status of patients’ reports, assessment changes to the concerned authority.
  • Ensure smooth functioning of admission, discharge and transfer procedures and assists others with patients’ activities and care with collaborative manner.
  • Follow handover-takeover process accurately with written/online notes.
  • Ensure introduction of next shift team member to patient and patient side medication check in each shift.
  • Executes physician’s orders for all assigned patients safely and properly administers medications and treatments. Strictly adhere read back policy (Refer policy no. 6 of medical admin).
  • Ensure identification safety for collection of specimens during collection and till the report informed to consultant.
  • Participate in consultant’s rounds and assist by giving relevant information based on nursing assessment/reassessment and observations.
  • Carry out routine check at each handover that all equipments are in working order.
  • Keep equipments in charging.
  • Ensure that all items used for patients inclusive of medications, surgical supplies are duly recorded and charged.
  • Ensure during internal transfer except the ordered medicine all other drug to be returned before shifting to other unit.
  • Maintain patients and accompanies safety by taking preventive measures against accidents.
  • Accurately records and reports incidents.
  • Ensure every error to be recorded.
  • Ensure strict infection control protocol and bio-waste guidelines to be followed for 24x7.
  • Ensure that daily briefing points and sign off are mandatory for day to day update.

Specific Unit Related Nursing Care:

  • Receives post-op Patient and also inform other departments.
  • Have basic knowledge of Patient Diagnostic procedure.
  • Observes conscious state and pupil reaction.
  • Draws Blood Sample (arterial and venous).
  • Assists in setting up ventilator, changes ventilator tubing and filter.
  • Assists in intubation and extubation and does ETT and oral suctioning.
  • Monitors and gives nursing care for patients on restraint.
  • Demonstrates ability and knowledge of adult and pediatric CPR.
  • Assists in Insertion of CVP and arterial line.
  • Performs chest physiotherapy and limb physiotherapy.
  • Assist in defibrillation.
  • Cares for surgical wounds and performs surgical dressing.
  • Maintains aseptic technique whenever required.
  • To follow Time Out for Invasive procedure.
  • IPSG/GCS – Prepare Nursing care plan, deliver and document.
  • Monitor and maintain the accurate I/O chart.
  • Monitor patient on ventilator.
  • Responsible to carry out – Critical Value Protocol.
  • Any other jobs assigned by superior.
