Position Purpose :

The Patient Support Assistant Nurse provides comprehensive nursing care in the home that assists patients to achieve optimum health and quality of life in situations of chronic disease. The Patient Support Assistant Nurse is responsible for the provision and management of effective nursing care to individuals and/or families in the community or in-Patient Support Programs.

Main duties and responsibilities:

• Monitoring the client’s health status and adjusting care plans as needed based on client needs
• Working with the family to ensure that they are comfortable with the care plan and know how to provide care if needed
• Scheduling appointments with physicians and other healthcare professionals to ensure proper treatment is being received
• Ensuring that the home environment is safe for the client by ordering new equipment or making repairs to items such as stairs or cabinets
• Assessing the client’s needs and working with the family to develop a care plan that meets those needs
• caregivers, helping with bathing and dressing, preparing meals, and administering medications
• Cultivating strong relationships with the client and his or her family in order to understand their needs and concerns
• Communicating regularly with case managers and other home care staff members about the client’s progress and needs

Educational Background and Experience

• A Degree in Nursing is required
