Description

Conduct initial and periodic assessments of patients’ abilities to perform ADLs and identify any needs or limitations.
Develop and implement personalized care plans focused on assisting patients with daily activities while promoting independence.
Regularly review and update care plans based on patient progress, changes in condition, and feedback from the healthcare team.
Oversee the delivery of ADL services, ensuring they are carried out according to care plans and patient preferences.
Provide assistance with personal care tasks, such as bathing, dressing, grooming, oral care, and toileting, when necessary.
Monitor and support patients during mealtimes, ensuring safe and appropriate feeding techniques, and assisting where required.
Assist with mobility support, helping patients transfer between beds, chairs, and other locations safely and comfortably.
Monitor caregiver performance to ensure adherence to care protocols and patient safety standards.
Provide ongoing coaching and feedback to improve the quality of patient care.
Ensure that caregivers are familiar with the specific needs and preferences of patients under their care.
