Description
- Conduct assessments of individuals' functional abilities, including mobility, self-care, communication, and socialization skills, to identify areas where assistance is needed.
- Develop individualized care plans in collaboration with clients, family members, and healthcare professionals, outlining specific goals, interventions, and strategies to address ADL needs.
- Assist clients with activities of daily living, such as bathing, dressing, grooming, toileting, eating, and mobility, providing hands-on support and guidance as needed.
- Encourage and facilitate participation in recreational and leisure activities, social events, and community outings to promote socialization, engagement, and overall well-being.
- Provide emotional support and companionship to clients, engaging in meaningful conversations, reminiscence therapy, and other activities to enhance quality of life and mental health.
- Monitor clients' health and safety, including vital signs, medication management, and fall prevention strategies, and report any concerns or changes in condition to healthcare providers.
- Maintain accurate and up-to-date documentation of care provided, including daily activity logs, care plans, and incident reports, in compliance with regulatory requirements and organizational policies.
- Collaborate with interdisciplinary team members, including nurses, therapists, social workers, and caregivers, to coordinate care, share information, and ensure continuity of services.
